Why Psychological Safety is Non-Negotiable
In the high-stakes world of military operations, the margins for error are razor-thin. As a Senior Noncommissioned Officer, I learned a sobering lesson early on: silence can be deadly. Whether on a battlefield, managing a multi-million dollar program, or sitting in a corporate boardroom, the most dangerous threat to a team isn’t always an external competitor or a technical glitch. Often, it is the internal fear of looking incompetent, intrusive, or negative. What is Psychological Safety?
Coined by Harvard Professor Amy Edmondson, psychological safety is the shared belief that a team is safe for interpersonal risk-taking. It means knowing you won’t be punished or humiliated for speaking up with ideas, questions, concerns, or—most importantly—mistakes.

The Lessons of the "Whack-a-Mole" Culture
Throughout my career transition from the U.S. Army to Program Management and Organizational Psychology Consulting, I’ve seen the same pattern repeat across every sector. In many organizations, as one colleague calls it, a "Whack-a-Mole" culture persists. When an employee sticks their head up to voice a dissenting opinion or point out a flaw in a project, they are metaphorically struck down by leadership. After two or three times, the "moles" stay in their holes and keep quiet.
The result?
The Boeing 737 Max crisis and Chernobyl serve as catastrophic reminders of what happens when employees are too terrified to challenge the status quo. The Boeing 737 MAX malfunction (2018-2020) and the Chernobyl tragedy (1986) are seminal examples of systemic failure, with corporate/organizational culture, poor engineering, and insufficient oversight resulting in devastating loss of life and reputation. Both instances were precipitated by pressures to prioritize speed, economy, and production over safety, resulting in devastating, preventable disasters.
Delayed Problem Identification: When people fear repercussions, critical information never reaches the decision-makers until it’s too late.
Innovation Stagnation: Creative breakthroughs require the freedom to fail. Without safety, people play it safe, and the organization plateaus.
Leadership Sets the Pulse
One of the most striking things I observe as a consultant is "the silence of the room." A leader asks, "Does anyone have any questions?" and is met with a deafening, dead silence. Leaders often mistake this silence for agreement. In reality, it is usually a sign of fear. As leaders, we must realize that people will mimic the behaviors we display. If we are defensive or dismissive, we send a clear signal: Just shut up.

Three Steps to Build a Safer, Stronger Team
Model Humility and Curiosity: Framework as a learning problem, not an execution problem. Admit your own fallibility to give others the "cover" to do the same.
Observe Before You Intervene: Watch your team’s dynamics. Who speaks? Who stays silent? How do you react when someone brings you bad news? The "messenger" should be thanked, not blamed.
Consistency is Key: Psychological safety isn't built with one speech or a clever slogan. It is built through repeated, predictable, and constructive engagement over time.
